Passporting

For Smartflight to schedule your passport online appointment with DFA, please follow this instruction:

1. Click this link https://onlineappform.passport.gov.ph
2. Choose proceed.
3. Then select your application type whether new, renew, or lost
4. Choose a specific DFA office where you would like to process your passport
5. Fill out the boxes of your details according to your PSA birth certificate and/or expiring passport
6. Download the fully filled out form
7. Then make a service fee payment of Php 1,000 either thru Smartflight’s scan to pay QR Code, GCASH at 09173323232, or bank-to-bank deposit or transfer to BDO Peso Savings Account NO. 001810393939 with account name Cherrie Anne A. Miguel
8. Email us at smartflightgm@gmail.com the downloaded form together with the service fee proof of payment (a photo of the deposit slip or screen shot of payment receipt from GCASH).
9. Notify Smartflight that you have availed our passporting services by texting us your full name, email address used, and date of your email at 0917 332 3232.
10. Expect within 24 hrs for a reply.  

NOTE:  Appointment process may take long depending on the slot availability of your chosen DFA office.
